2. Current Bio Information (Who they are, What they have done in their field)
3. A photo of Nominee Candidate
4. Contact Information for Nominee Candidate (email, phone # or fb)
Only nominations received 12/7/14-1/5/15 with all requested information and the category in the email subject line will be counted towards an artist/individual’s total for consideration in nominations
